Tender description

The purpose of this service is to offer a programme of assessment and support to meet the specific needs of young carers aged 5 to 18 and their families in Merton. All children referred will be assessed in accordance with recent legislation and statutory guidance. Assessments will determine level of need and support offered with priority given to those at enhanced or specialist levels on the child well being model using a partnership model. Assessment and Review of Casework — all young carers referred will be assessed in line with the Children and Families Act (2014) where assessments will include an analysis of whether it is appropriate for the young carer to provide, or continue to provide, care for the person in question, in the light of the young carer's needs for support and other needs and wishes. Assessments will be carried out in line with ‘No Wrong Doors’ (2015 — template Memorandum of Understanding between Adult and Children's Social Care) using a ‘Whole Families Approach’. Assessment and review of young carer's needs will ascertain individual support packages for young carers. Support offered will be delivered in line with priority outcomes as defined in the Carer's Strategy.
