Tender description

The Support to Live at Home service is for: twenty four hour home care provision for older and disabled people (including those funded by Health for ongoing long term health conditions); end of life care at home; dementia care; and reablement. The work does not include: provision for people with a learning disability; provision for people with poor mental health; or provision for people funded by health and social care due to complex needs. There are approximately 2900 home care hours delivered in this area currently per week. The Support to Live at Home contract will be a spot purchase contract. The geographical area in question for the delivery of home care comprises: Wraxall, Tickenham, Nailsea, Yatton, Congresbury Long Ashton, Failand, Backwell, Dundry, Felton, Winford, Redhill, Blagdon, Wrington, Langford, Churchill, Sandford, Winscombe The Provider is required commence all new Packages and to TUPE transfer and provide the Council's In House reablement team START's existing work from the Commencement Date of the Support to Live at Home Contract, 1.7.2017. Independent Sector Provider existing Packages will be transferred to the Provider on a phased basis from the Commencement Date over an approximate 6 month process in agreement with the Council. There are approximately 2900 home care hours delivered in this area currently per week.
