Tender description

The contract is for the provision of Reablement Services to individuals in their own home or the community following an assessment with a responsible social worker, care co-ordinator or health professional. Reablement is a short-term intervention service, offered for a maximum of 6 weeks and is designed to help people adapt to a recent illness or disability by learning or re-learning the skills necessary for independent living at home. The service may be offered to someone who has recently been discharged from hospital and can include the provision of personal care, help with daily living activities and other practical tasks. Reablement encourages service users to develop the confidence and skills to carry out these activities themselves and continue to live independently at home. Reablement Services in County Durham are currently provided by both the DCC In-house Provider County Durham Care and Support (CDCS) and an independent sector Provider, with CDCS having the larger market share. However, this procurement exercise is to replace that mixed model arrangement with one reablement Provider from 1.4.2017. The duration of the contract is 4 years, subject to the right of the Authority (at its sole discretion) to exercise its right to extend the contract by up to 24 months (2 x 12 month extension periods). The maximum length of the contract is therefore 6 years.
