Tender description

Rochdale Borough Council is procuring a Community and Family Prevention Service that will form an integral part of the Borough's prevention offer as set out in the Locality Plan. The aim of the service is to provide a programme of interventions across the Borough that empower people to make healthier lifestyle choices, connect with their community and generally improve resident's well-being to prevent the escalation of long term health problems. The contract term is expected to be three years with the possibility of an annual extension for a further 2 years. We welcome single provider, consortia bids & other collaborative approaches as we look to develop the value and diversity offered by small, mid-sized and larger organisations. This marketing event is an opportunity for individual providers to consider such options and hear about our vision for this service. The aim of the service is to provide a programme of interventions across the Borough that empower families to make healthier lifestyle choices, connect with their community, support volunteering and improve mental well-being. This will help to prevent long term health problems. The service will ensure that we address our local priorities of childhood obesity, tobacco use, reducing cardio vascular and the risk of long term conditions and will increase uptake of preventive services particularly for those in greatest need. The targeted interventions delivered by the service will be aimed at people: With multiple lifestyle related risk factors such as high blood pressure, high cholesterol etc. With disabilities that contribute to an increased risk of health risk factors.
