Tender description

Nottinghamshire County Council (the ‘Authority’) on behalf of ‘the Commissioners’: Nottinghamshire County and Nottingham City Councils and NHS Nottingham City, Mansfield & Ashfield, Newark & Sherwood, Nottingham West, Nottingham North & East and Rushcliffe Clinical Commissioning Groups, is establishing a Dynamic Purchasing System (DPS) of organisations able to provide home based care & support services for: — Older adults — People with dementia — People who may be physically frail — People with physical disabilities — People who are at the end of life — People with learning disabilities/other cognitive difficulties — People with mental health needs Services may be procured under the DPS by the Commissioners individually, collectively or in any combination. Providers must have experience of providing care and/or support services relevant to the DPS. They must, where required by legislation, be registered with the care regulator & comply with relevant regulatory standards. Contracts may be advertised under this DPS for any of the following: — Provision of on-going domiciliary care — Services which enable prompt hospital discharge — Services to prevent unnecessary admission to hospital or care home — Care services in Extra Care housing settings — Short term Assessment work within Extra Care or other housing schemes — End of life services — Bespoke care packages — Care for elderly or disabled people within a prison — 24 hour live- in care — Carers' Crisis Prevention — 24 hour response service — Services arranged through Individual Service Funds — Any other Home Based Care and Support Services which contribute to delivering the aims and objectives detailed in the specification Contracts may be configured on a County-wide basis, in more localised geographical models, or designed in other appropriate formats.
