Tender description

Application to be included on the Dynamic Purchasing System (DPS) for NHS Health Checks in Worcestershire. The NHS Health Check programme began in April 2012. The aim is to provide NHS Health Checks for everyone between the ages of 40 and 74 who meet certain eligibility criteria. An NHS Health Check will detect and prevent heart disease, stroke, kidney disease and diabetes and will support people to reduce or manage that risk through individually tailored advice. The Council invites suitably qualified organisations who meet the criteria outlined in the Service Specification to submit applications to be placed on the DPS from 1.4.2017. Contracts will be for a period of 2 years with an option to extend for up to 4 further years, subject to performance reviews. Any questions relating to the information in the Application Pack should be sent by e-mail to Samantha Davey, Commercial Contracts Officer via In-tend and must be received by 16:00 on Wednesday, 2.11.2016. Telephone questions will not be accepted. The Council will publish a summary of the questions received, together with the Councils' response to the questions, on In-tend by or on Friday, 7.11.2016. To submit an application please complete and return the Application Questionnaire in accordance with the completion guidance. Initial applications must be received by the Council by 16:00 on Tuesday 18.11.2016. After this date the DPS will remain open and questions can be submitted at any time.
