Tender description

Sensory Impairment services in the City of York are currently delivered through a combination of internal and external arrangements. Existing service delivery is split across pathways and providers and there is a desire for greater co-ordination and integration around service provision, allied to consistent practice in the identification, recording and assessment of people with sensory impairment(s). City of York Council therefore wishes to procure a single point of access Sensory Support Hub to provide a range of statutory and non-statutory Sensory Impairment Services for adults residing in the City of York; including — Comprehensive information and advice that helps to prevent, reduce or delay the need for ongoing care and support. — Support with registration (and information about the benefits of registration). — The offer of a proportionate assessment. — Support Plans (and regular reviews of support plans) for customers receiving an ongoing intervention through this service. — Rehabilitation Services. — Access to equipment to support daily living. — Support specifically tailored to Deafblind people unable to benefit from those services aimed primarily at Visually Impaired or Deaf/Hard of Hearing customers. — In addition the Service Provider will need to demonstrate added value by either directly providing or referring adults with sensory impairment(s) to the following types of support services: — Volunteering and befriending Services. — Equipment Demonstration and Resource Centre(s). — Counselling and emotional support Services. — Employment and welfare support. — Education and training. — Social and leisure activities. — Community and peer support. — Other initiatives and forms of support.
