Tender description

Scope. The NHS North West Surrey Clinical Commissioning Group (NWS CCG) is responsible for commissioning healthcare services for around 360 000 people in North West Surrey across the boroughs of Elmbridge (West), Runnymede, Spelthorne and Woking. NWS CCG seeks to procure a range of community healthcare services for adults (largely defined as aged 18 years plus). The services will deliver a proactive approach to care that identifies and supports vulnerable people in the community, prevents serious illness and provides timely coordinated care in a way that integrates out of hospital care and support systems. The Provider will be required to work with the wider health and social care providers to develop and implement the model of care and to enable a fully integrated service to be delivered, including through the future development of alliance arrangements with Partners and NWS CCG. This fits with the CCG's vision of creating an accountable care organisation. Core principles North West Surrey CCG has a vision for the development of a comprehensive and fully integrated model of health and care provision to our local population and Bidders will be required to demonstrate their commitment to this vision. Integrated community services will work under the leadership of General Practice to provide a holistic health and care response genuinely tailored to the needs of a specific individual. Our new model of care, evolving from 2017 onwards will require the Provider to focus on the creation of an organised, coordinated and effective Out of Hospital provider environment that is seen as the main conduit for meeting a person's health and care needs. The Out of Hospital environment will work on a level playing field with high-quality, specialist services in the main acute trust — Ashford and St Peter's Hospitals NHS Foundation Trust, Surrey and Borders Partnership Trust (SABP), Adult Social Care and Primary Care to develop cross system pathways, build clinical networks and share expertise and advice to the ultimate benefit of the patient. The transformation of community services to realise this ambitious model of care will take place in line with a number of underlying principles: — Alignment of community healthcare services with primary care leadership through alliance based joint working arrangements. — Development of Locality Hubs which are core, integrated teams focussed around clusters of GP practices and working directly with primary care professionals on a day to day basis. — Robust care coordination and the use of key workers to coordinate access to different parts of community services, mental health services, the voluntary sector and social care, building on the role of the ‘trusted assessor’. — The development of a much higher level of generalist skill across community nursing rather than specialised silos. — A single point of access for referral and telephone access. — Interoperability with all health and social care systems and streamlined, efficient methods of referral and information sharing. — A service that responds to the needs or the patient with regards to response times and operating hours. Term NWS CCG seeks a Provider to deliver adult community healthcare services from 1.4.2017 for a period of not fewer than 5 years. The indicative financial envelope is 20 276 000 GBP per annum, making an indicative envelope of 101 380 000 GBP over a 5 year term. Services The indicative services to be procured include: — Community nurses; — Specialist nurses to support core functions; — Physiotherapists; — Occupational therapists; — Dieticians; — Podiatry; — Speech and language therapists; — Care coordinators. The community services contract currently in place includes both children's and adult services and serves other CCGs. For the avoidance of doubt, this procurement covers only adult community healthcare services for NWS CCG. Other procurements and arrangements are planned for children's community healthcare services and other CCG areas. Procurement process NWS CCG is following a Competitive Procedure with Negotiation whereby qualified Bidders will meet with the CCG to discuss innovative responses to the proposed model of care and to demonstrate how they will work with the NWS CCG and planned Neutral Partners to move towards an outcomes based contract. The procurement process for this Procurement is planned to include: Stage 1: Pre-Qualification Questionnaire; Stage 2: Initial Proposals from pre-qualified Bidders; Stage 3: Final Proposals from shortlisted Bidders. After the PQQ stage, the CCG plans to introduce qualified Bidders to Neutral Partners. The CCG is working to secure Neutral Partner status for local GP Federations, local acute providers, local mental health providers and local ambulance providers. This means that it is planned that the proposed Neutral Partners will not enter into separate commercial relationships with Bidders and will not support 1 Bidder over another, but will be available to discuss plans with Bidders at scheduled sessions.
