No Pressure Blood Pressure Campaign INTRODUCTION Camden Clinical Commissioning Group (CCG) is a local organisation which 'buys' healthcare services for Camden. We are governed by member GP practices and lead the development of the local healthcare system with our partners and the people of Camden. Over the last few months we have been working with the Design Council on a key target; reducing the number of unnecessary clinical episodes by proactively diagnosing hypertension before a critical event takes place such as a stroke or heart attack. PROJECT OVERVIEW There are over 18,000 patients at risk of hypertension in Camden whose condition could be better managed if they were identified before an acute healthcare episode occurred. The challenge is to develop a holistic social marketing campaign targeted at patients and staff of the 36 Camden GP surgeries to proactively measure the blood pressure of individuals at risk of hypertension. GP surgeries have devices (called surgery pods) enabling patients to independently measure BP and are financially remunerated for doing so. The challenge has two key elements: • Opportunistic: Promote all patients above the age of 40 years to independently measure BP using a surgery pod available in the GP surgeries. • Targeted: Connect with patients at risk on the Camden GPs register and invite them to measure BP at their surgery or through the use of an ‘over-the-counter’ BP cuff (issued by the GP Practices) and smart phone DESIGN OPPORTUNITIES Following two user group workshops held in November and December with key Camden CCG and GP Practice staff a variety of potential solution areas were identified; Opportunity 1 Digital 1. Alert individuals when they book online that they need to check their BP 2. Text individuals who need their BP checked to come into their practice 3. Enable alerts when patients self-check-in 4. Send letter to patients to check BP 5. Mobile phone App for patients linked to patient account Opportunity 2 Training 1. Educate staff how to action protocols and alerts related to measuring BP 2. Education of clinical and non-clinical staff on importance of measuring Opportunity 3 Support 1. Link to Pharmacies, many of which also have BP Pods 2. Use the healthcare advocates/patient assistants to promote the surgery pod and help patients Opportunity 4 Environment/Information 1. Clear, concise information on how to use the BP Pod 2. Information in practices to promote using the BP Pod 3. Improve the physical environment for BP Pod offering more privacy Opportunity 5 The ‘Pod’ 1. Update BP Pods to newer models
