North of England Commissioning Support (NECS) working for and on behalf of NHS Sunderland Clinical Commissioning Group (CCG) wish to engage with potential providers through a Request for Information (RFI) process in relation to a proposed procurement of Sunderland Practice Based Medicines Optimisation Support. The purpose of the Practice Based Medicines Optimisation Support is to provide comprehensive, high-quality medicines optimisation service to member practices that includes delivery of the outcomes of the SCCG practice medicines optimisation strategy, and the annual work plan. The objectives are to ensure: • To improve the quality of care and patient outcomes from medicines • To support people with their medicines to empower them to improved self-care and management of long-term conditions • To reduce risk to patients from their medicines or from sub-optimal use of medicines • To improve the cost-effective use of medicines to ensure best value for money for the NHS and release resources for improved patient care • To provide expert advice to staff in – or supporting - general practice on medicines optimisation, prescribing and pharmaceutical issues. • To work in an integrated manner with the practices ,CCG and other primary care services to become an embedded member of the multi-disciplinary primary care team • To support practices to access community pharmacy delivered services for their patients, such as medicines use reviews to improve patient care. The CCG’s are seeking to obtain input and feedback from provider organisations as part of the development of a service specification and service model for delivery of a Practice Based Medicines Optimisation Support.
