Tender description

Provision of an Integrated Care Centre Service for patients in Herne Bay. The service will provide: — A locally accessible integrated model to include access for minor injury/illness including x-ray diagnostics, enabling easy and timely access to urgent health services, ensuring patient safety. — Access to a range of community based services to include DVT, catheter care, wound clinic, musculo-skeletal services, and frailty clinic. Provision of an Integrated Care Centre Service for patients in Herne Bay. The service will provide: — A locally accessible integrated model to include access for minor injury/illness including x-ray diagnostics, enabling easy and timely access to urgent health services, ensuring patient safety. — Access to a range of community based services to include DVT, catheter care, wound clinic, musculo-skeletal services, and frailty clinic. The contract will be for an initial 3 year term, with an optional 2 year extension. The contract value covers the whole 5 year term.
