North of England Commissioning Support (NECS) working for and on behalf of NHS Sunderland Clinical Commissioning Group (CCG) wish to engage with potential providers through a Request for Information (RFI) process in relation to a proposed procurement for Sunderland practice-based medicines optimisation support. The purpose of the practice-based medicines optimisation support is to provide comprehensive, pharmacist –led, high-quality medicines optimisation service to member practices that includes delivery of the outcomes of the SCCG practice medicines optimisation strategy, and the annual work plan. The CCG is seeking to obtain input and feedback from provider organisations as part of the development of a service specification and service model for delivery of a Practice Based Medicines Optimisation Support.
