Tender description

To support service personnel in transition from serving to civilian life NHS England is looking to secure providers to deliver a service for those armed forces personnel in transition. The pathfinder service will adopt a partnership approach with the MOD to provide a transition service for serving personnel who are approaching discharge in order to: — Ensure continuity of care where mental health issues have already been diagnosed and care plans are in place — Develop an agree care plans for those service personnel who are transitioning and who are identified as needed mental health support in transition — Co-ordinate and provide support across agencies for vulnerable individuals in transition. Providers must be capable of delivering the service across a wide geography which may include through a lead or prime contractor model. Providers must also demonstrate how they will link with appropriate and relevant third sector organisations to ensure that the needs of veterans are met. Lot 1 — North. Please see ITT documentation for details on geography for this Lot. Following this procurement process, the Contracting Authority intends to award one contract for each of the Lots described above. Further information on the proposed structure of the contract is provided in the ITT documentation made available online to interested parties on the e-tendering portal. The contract/s let as a result of this procurement to deliver the Services will be 5 months (up to 31.3.2017) followed by an optional extension period of 6 months (up to 30.9.2017). Therefore if all extension options are used, the maximum (extended) contract period will be 11 months. The contract values provided below relate to the maximum contract values for all Lots combined and including any available extension options. Please see the ITT documentation for further details on contract values for individual Lots. Lot 2 — Midlands and East. Please see ITT documentation for details on geography for this Lot.
