Provision of an Equipment Supply, Managed Telecare and Assistive Technology Service for adults living in East Sussex Overview of Service below: East Sussex County Council Adult Social Care is re-commissioning a new service model to provide our county-wide Telecare service. The purpose of the Equipment Supply, Managed Telecare and Assistive Technology Service is to pro-actively support adults to maintain and develop their independence through the increased use of Telecare and AT as part of the Departments move towards re-ablement. Service components include: — Equipment supply, — A cost-effective managed installation service, supported by an online ordering facility, — An efficient call monitoring service that enables the safe care and early detection of increased client need, pro-actively supporting clients to increase their ability to self-care, — Support to Adult Social Care prescribers to increase client independence and reduce the need for other forms of support.
