Tender description
The purpose of this Prior Information Notice (PIN) is to inform the market of the forthcoming planned procurement for: The Provision of End of Life Healthcare Services for Mid Nottinghamshire. Mid Notts Clinical Commissioning Groups (CCGs) has redesigned their approach to End of Life (EOL) services to ensure delivery of an integrated and responsive model which builds on good practice and seeks to innovate and transform the care pathways for patients at end of life. The CCGs are seeking a Lead Provider or consortium of providers solution to deliver a number of EOL services operating in synergy to provide joined up, holistic EOL care for patients. The contract will incorporate the provision of a Co-ordination Hub and Urgent Response Service available 24 hours a day 7 days a week. It will also include provision for palliative care, physical, psychological, spiritual and social support to patients, families and carers. This new service provision is expected to be in place and fully operational by 1.4.2017.
