Tender description

Haringey Clinical Commissioning Group (HCCG) and London Borough of Haringey (LBH) are currently reviewing support services for individuals who have experienced and survived stroke with a view to recommissioning a new service by April 2017. The service will support people and their carers to navigate the services and support available across the system and work in close partnership with a wide a range of statutory and non-statutory agencies in order to ensure improved outcomes and user experience Keywords: Stroke, Support
