Adults short term home support — The Service will provide a personalised short term service to support Service Users to retain or regain their independence. The Service will be flexible and practical, to enable a Service User's safe transition from hospital to empower them to remain independent at home. This will be tendered as 3 lots based on 3 zones which are aligned to 7 CCG GP clusters. A map within the ITT document will provide more information on which areas are covered by which lot.
