Tender description

The County Council wants to commission a county-wide Service which provides a range of support to people with dementia or memory problems, their carer and family. The Service is split into 3 lots: Lot 1: Activity, Information and Infrastructure: — Single point of access for Lot 1, 2 and 3, with basic advice and information, — Work with communities, providers and other partners to make services accessible to people with dementia/ memory problems and build community resilience, — Provide a range of mainly group based, local, community services; Lot 2: Specialist Dementia Support: — Time limited, 1:1 support offered to every person diagnosed with dementia (and their carer), — Ongoing source of 1:1 support/ advice/ navigation for any carer/ person with dementia/ MCI whose needs cannot be met by Lot 1, — Pro-actively support carers/ people with dementia identified as ‘high risk’; Lot 3: Community Dementia Specialist Nurses (Start date 1.4.2016): — Intensive, preventative mental health support for a small cohort of carers at, or close to, crisis. The requirements for each lot have been developed as a result of feedback from carers, people with dementia, people with mild cognitive impairment (MCI), front-line social care staff, GP's and providers. It also addresses the priorities and objectives set out in Hertfordshire's Dementia Strategy 2015-2019. It is anticipated that significant partnership and collaborative working will be required to deliver this Service successfully to all areas of Hertfordshire. The Contract will be for an initial term of 2 years, with the option to extend for a further period of up to 36 months. The total annual funding across all 3 lots is approximately 1 457 000 GBP: Lot 1 — 636 000 GBP; Lot 2 — 653 000 GBP; Lot 3 — 168 000 GBP. The estimated annual range across all 3 Lots is 1 310 000 GBP-1 610 000 GBP. This procurement is an Open, 1-stage process. Tenderers wishing to take part in this project are invited to ‘express interest’ which will give access to the full procurement documentation in the e-tendering system. Tenderers should be aware that due to the nature of the Services provided, any Contract, for Lots 1 and 2 and 3, formed as a result of this procurement process shall be executed as a deed and attested under the common seal of the Council. Tenderers should seek independent legal advice on the implications of this prior to submitting their Tender, where appropriate. Please be aware that TUPE applies.
