Tender description
Kingston CCG (CCG) and the Royal Borough of Kingston (RBK) are committed to delivering the Kingston Coordinated Care programme which aims to: • Support the development of active and supportive communities in which people are enabled to stay healthy and well, living independently as part of a thriving and resilient community; • Develop truly customer-centred care that supports people with complex needs to achieve the best possible quality of life and the goals that matter to them with an increased focus on prevention, proactive care, and self-reliance; and • Demonstrate savings and efficiencies for the CCG and RBK whilst delivering high quality of services to customers. These aims will be achieved by the following objectives: 1. Putting in place a customer centred approach to everything we do that complements our current analysis and understanding of people’s needs; 2. Building resilience in Kingston’s communities so that people can remain independent and healthy for as long as possible, and are able to access a variety of community support options if and when needed, and; 3. Designing and implementing a new model of integrated services that deliver what is needed by people in Kingston, and are simpler, streamlined, and cost effective. As part of delivering these objectives, the CCG and RBK are undertaking a provider engagement exercise in order to understand: • The potential level of interest from providers in delivering a new model of integrated care for Kingston; • Views and insights that providers may have on delivering an integrated care model for Kingston • The experience and capability of delivering integrated care and any innovations providers have implemented as part of this Undertaking the provider engagement will enable the commissioners to gather information that will help us to understand options available when it comes to commissioning a new model of care. This soft market testing activity does not constitute a call for competition to procure any services, supplies or works for the CCG and RBK and the CCG and RBK are not bound to accept any proposals offered. The CCG and RBK are not liable for any costs, fees or expenses incurred by any party participating in the soft market testing exercise.
