Tender description
Health Needs assessment of Care Delivery Group 4 Nottingham City has 306,700 people living in the area and 365,000 people are registered with a Nottingham City CCG GP practice. The population is diverse and multi-cultural, with high rates of deprivation and poor health. The biggest killers in the Nottingham City area are cancer, respiratory disease and cardiovascular disease. The City has been organised into 8 Care Delivery Groups (CDGs) which are geographically aligned to local authority boundaries and have varying health needs and levels of deprivation. Nottingham City CCG is seeking a provider to lead on the completion of a full primary medical health needs assessment within the CDG 4 area. This area has a registered population of 70,384 and is relatively deprived, with a significantly lower life expectancy than the East Midlands average. BME groups form a high proportion of the population in this area. This primary medical health needs assessment will help to determine the needs of the local population and identify any needs that are not presently being met by primary medical services. The provider will be expected to produce a report on this assessment by 31st October 2016. In line with optional Clause 14 of the NHS Terms and Conditions of Contract for the Supply of Services please note that any Intellectual Property, Analytical data or other output shall be owned by the Authority and any such Intellectual Property or Data is only to be used by the Supplier upon approval by the Authority. The assessment will involve interviewing stakeholders, data gathering and analysis of information from multiple sources within the specified CDG which may include for example: • GP Practices • CQC reports • Service Commissioners • Crime and Drug Partnership • Social Care • Public Health data • Secondary and community services data Providers must outline their approach to undertaking the assessment, skills and experience of both their organisation and team members along with a full schedule of work for completion within the contract period. Bids must be received by 17th August 2016 and the successful applicant will be informed on 19th August. The provider will meet Nottingham City CCG commissioning Team on the 23rd August at 11.00am.
