Barnsley Clinical Commissioning Group (CCG) is looking for a supplier to deliver an Ophthalmology Service. Barnsley CCG is committed to the provision of high quality, dedicated and professional ophthalmology services for patients with eye conditions, centred on clinical assessment and treatment by getting the patient onto the appropriate management pathway. The primary objectives of the service are: — To deliver ophthalmology services in a safe and effective manner for the population of Barnsley; — Provide holistic care for all adults referred to the service, designing and delivering tailored care that includes health promotion and early prevention, to meet individual health needs; — Work in an integrated and partnership way with primary, secondary, social care and the independent and voluntary sector care; — Enable patients with long term and degenerative conditions to continue to live as independently as possible; — Sensitive and timely communication to all individuals, and where appropriate those important to them, about their wishes, preferences and decisions regarding their care and treatment; — Ensure that service delivery is high quality, responsive, high performing, provides value for money, is environmentally efficient and innovative in the use of information technology.
