Tender description

Kingston Coordinated Care (KCC) is a shared Kingston CCG (CCG) and Royal Borough of Kingston (RBK) programme to transform the provision of health and social care in Kingston. KCC aims to commission high-quality holistic services which are relevant to local needs, focussing on: — Supporting the development of active and supportive communities, promoting healthy, independent, resilient and thriving communities; — Developing customer-centred care supporting people with complex needs with an increased focus on prevention, proactive care and self-reliance. — Designing and implementing a new model of integrated services that deliver what is needed by people in Kingston, and are simpler, streamlined, and cost effective. As part of delivering the objectives, the CCG and RBK are undertaking a provider engagement exercise in order to understand: — The potential level of interest from providers in delivering a new model of integrated care; — Views and insights that providers may have on delivering an integrated care model; — The experience and capability of delivering integrated care and any innovations providers have implemented; — Areas for potential savings and efficiencies. Undertaking the provider engagement will enable the commissioners to gather information that will help us to understand options available when it comes to commissioning a new model of care.
