Tender description

Swindon Borough Council (the Authority) is seeking to appoint a service provider to coordinate manage and deliver (both directly and indirectly) domiciliary care and community support services in Swindon (the Project). The successful provider (Lead Provider) will be required to transition the provision of the services to service users from a traditional time/task service model to an outcomes-based service model and ultimately to provide the services on the basis of an agreed Capitated Budget (see Section II.2.4)). The term of the Contract will be 10 years subject to contract reviews at years 5 and 8 where the Parties will have the option to make changes to the Contract or terminate the arrangements. The Project will consist of 3 Phases: Phase 1 — Provision of the Services on a time/task model; Phase 2 — Transition of the services to an outcomes-based model; Phase 3 — Delivery of the services by way of a Capitated Budget. The Capitated Budget will involve payment by the Authority in advance of an agreed budget to the Lead Provider to deliver Services to specific population groups. The Capitated Budget will be agreed during the term of the Contract. See the procurement documentation for more details. Modifications of the arrangements under the Contract may be made during its term to facilitate implementation of the Capitated Budget and the scope of the Contract may be extended to include additional services (see procurement documentation). The estimated contract value includes such services. Any modifications shall be strictly in accordance with Reg 72 of the Public Contracts Regulations 2015 and shall be effected in accordance with the change procedures set out in the Contract.
