Rapid Response Domiciliary Care - prevention of hospital admission and timely discharge service Cheshire East Council is inviting applications from suitably experienced providers of domiciliary care to deliver aservice that will complement that being delivered our existing providers. Through delivery of a rapid response service, with an enabling approach, the aim is to prevent unnecessaryhospital admissions and support the prompt discharge of patients from hospital back to their homes and avoid unnecessary longer term care packages where appropriate.
