Tender description
NHS Camden CCG is looking to engage with a suitably qualified provider to undertake the Frail and Elderly Programme and patient Engagement and Insights Project. The contract value for this programme should be between £45,000 and £50,000 excluding VAT Camden has a relatively young population. In absolute numbers, people aged between 25 and 40 years old make up the largest age group. Although older people make up a relatively small proportion of Camden’s population (25,000 persons aged 65 years old and over), the highest percentage growth in the next 10 years will be seen in those aged 75 years and older. Frail and elderly people are a health priority in Camden for the following reasons; • By 2022, there will be a 35% increase in people aged over 85 • Rising numbers of older people with long-term conditions and mental health disorders • People over 65 account for 39% of all A&E admissions to hospital and 60% of all emergency bed days • Increasing costs caused by unplanned and uncoordinated care often leading to avoidable admissions to hospital The overall frail and elderly programme aims to improve the quality of life for all frail and elderly patients and their carers through (1) early identification and assessment, (2) preventing the delay and onset of frailty, (3) increasing the amount of time people spend at home through early intervention, prevention and recovery, (4) a high quality of care, equity of access to the right services at the right time, (5) reducing the amount of time spent in hospital and (6) an overall integration of health and social care services around the patient. The programme consists of several projects and approaches that support these aims. The Frail and Elderly programme seeks to promote excellent communication and collaborative relationships with staff, patients and carers; and to improve the measurement of patient outcomes and capture both the patient and carer experience. This project will be commissioned jointly between Camden Clinical Commissioning Group and the London Borough of Camden to identify engagement approaches and options to develop tested solutions and an innovative engagement and insights toolkit.
