Nottingham City CCG invites professional qualified suitably experienced providers to evaluate the effectiveness of the Health & Social Care Transfer to Assess & Care Home Vanguard Beds Pilot models. The successful provider will be required to determine • Whether and how the pilots have achieved their objectives? • What impact the pilots have on the lives of the individuals receiving the services? • What works well and what doesn’t work so well and why? This should consider the clinical models and input from different professions. • Whether the pilots have improved health and social care outcomes (measurable and actionable) in the short term? • Whether they have reduced costs and improved system efficiencies? • Whether/how the model supports the recommendations within the new NICE clinical guidance (NG27) “Transition between inpatient hospital settings and community or care home settings for adults with social care needs”.
