Tender description

This Procurement is being conducted as a 1 stage process and will be carried out by NHS Shared Business Services on behalf of NHS Heywood, Middleton and Rochdale Clinical Commissioning Group. The service aims and objectives are to: — Deliver a high-quality, responsive, cost effective anticoagulation therapy monitoring service, for patients aged 18 and over, who require short-term or long-term (often lifelong) prescription of a Vitamin K antagonist (e.g. warfarin, sinthrome). — Prevent stroke or uncontrolled bleeding through effective anticoagulation control. — Deliver anticoagulation therapy prescribing and monitoring in accessible community settings i.e. close to patient's homes, or in patients' homes where the patient is housebound or has short-term mobility problems. — Deliver lean and clinically-evidenced treatment pathway for patients. — Encouraging patients to self-manage their coagulation and treatment compliance.
