Tender description

NHS Hounslow Clinical Commissioning Group (HCCG) intends to commission a Primary Care Patient Coordinator Service. The CCG would like to invite providers with an appropriate level of capability and capacity to express their interest in an opportunity to deliver this service. The aim of the Primary Care Patient Coordinator Service is to support the delivery of care for patients registered with Hounslow CCG GP Practices by providing coordinating staff to enhance and improve pro-active care, prevention and self-care. The anticipated annual contract value is circa 660 000 GBP. Should HCCG decide to undertake a formal procurement process, this is anticipated to commence in October 2016 working towards full service commencement early in 2017. The responses to this Market Engagement invitation will inform HCCG's commissioning intentions.
