Tender description

Lot 1 — to provide care and support to individuals identified by the Health and Social Care Partnership (HSCP) as requiring a level of support to meet their planned outcomes; Lot 2 — Self Directed Support Providers (Option 2) — Details of providers appointed onto this lot will be made available to a service user who has instructed the Council to make arrangements on their behalf, but wishes to exercise choice over how their relevant budget is used to support their outcomes. To provide care and support to individuals identified by the Health and Social Care Partnership (HSCP) as requiring a level of support to meet their planned outcomes. Contract commencement is expected to be 3.10.2016 or other date as advised by the Council. Award is offered for a period of 24 months with 2 further 12 month extensions available. The option to extend the Framework Agreement will be at the sole discretion of the HSCP and will be dependent on a continuing requirement for the service and contract performance. It will be taken that bidders responding to this opportunity have read, understood and comply with the Tender Information Pack contained with the PCS-Tender system (see attachments). This document contains the terms and conditions of award. All bidders responding must also be registered with the Care Commission and have all necessary polices and procedures in place to allow them to adequately manage this contract if successful. The Council reserves the right to annually refresh lot 2 under the terms of a Dynamic Purchasing System.
