Tender description

The development of person centred coordinated care is a key building block in Southampton’s Better Care Plan. The plan has refocused attention of the proactive or preventative agenda for health, care and wellbeing and provides the main driver for the commissioning of the Community and Wellbeing Team. This service will encompass those aged 18+ registered in Southampton City who have a specified need for proactive care, where this is not already provided by other core and specialist community services. Aims: To reduce the need for acute, urgent and long-term care by proactively identifying those at risk earlier to ensure the correct support is in place in a timely way. To promote this approach by being part of a wider network of health, care and wellbeing delivery across the city. This may include involvement in local integration projects for the prime provider and other better care schemes implemented by the commissioner.
