Tender description

The contract is for a period of 3 years with the option to extend for a further 2 years and will commence 1 April 2017. The service will be delivered to people living in twenty (20) self contained flats within an extra care scheme provided by the Commissioner through the Registered Social Landlord Moss Care. The scheme has a large communal lounge and an evening meal is provided as part of an individual’s tenancy by an on site caterer. The service will deliver personalised support that offers an enabling environment for individuals, thus giving people more opportunities to develop pathways to independence with a view to becoming less dependent on support in the future. The Provider will deliver support to Service Users on a twenty-four hours per day basis and 365 days per year. There will be sleep-in duty each night, 365 days per year that will typically cover the hours 11.00pm to 07.00 am. This will be covered by one member of staff. The times and length of the sleep in duty may vary to ensure individual’s needs are met outside of these times. The Provider will have a flexible approach to the delivery of the sleep-in duty. Currently there are approximately 312 commissioned hours provided per week this is split into two components; 200 Core Hours and 112 Individual Hours. This figure can alter over time dependent on assessed Service User need. Core Hours are the minimum number of hours that the Commissioner guarantees to purchase from the Provider. Individual Hours are the hours purchased in addition to the Core Hours to meet the assessed needs of the Service User. In addition to the provision for the 20 flats within the extra care scheme, the Provider will deliver a service to an assessment flat facility which is located within the building. The Provider will deliver the support an individual requires to access the assessment flat as identified in the Commissioner’s assessment and these hours will be in addition to the overall commissioned hours for the tenants living in the scheme. The service will be delivered as and when required and there is no guaranteed usage of this facility. There will be an expectation that the Provider will actively liaise with other agencies involved in the support delivered to individuals. In addition the Provider will promote a culture that develops links in the community to support individual’s to develop opportunities that enhance their quality of life and reliance on traditional paid support.
