RE: TENDER FOR THE PROVISION OF PERSONALISED EXTRA CARE SUPPORT FOR PEOPLE WITH A PHYSICAL AND/OR SENSORY DISABILITY AGED 18 - 55 The service will be delivered to people living in twenty self-contained flats within an extra care scheme provided by the Commissioner through the Registered Social Landlord Moss Care. The scheme at Lomas Court, Ashton-Under-Lyne, has a large communal lounge and an evening meal is provided as part of an individual’s tenancy by an on site caterer. There will be charges to the support provider for access and utility costs for staff facilities such as sleep in and staff room and this is currently in the region of £1,800 per annum. The provider will work in conjunction with the Landlord to support individual’s to understand and maintain their rights as a tenant. This will include information sharing protocols and regular meetings to ensure the best delivery to the service users. Contract term - 3 years with the option to extend for up to 2 years.
