Tender description

London Clinical Commissioning Group (CCG) commissioners seek applications for the provision of continuing healthcare services in nursing homes. The Any Qualified Provider (AQP) process covers service users eligible for continuing healthcare mental health adult (including dementia) and continuing healthcare physical disabilities adult (including frail elderly) including end of life care.
