Tender description

The Care Together programme has been brought about by what has been referred to as a perfect storm. A combination of need to improve the health indices for the population of Tameside and Glossop, drive up quality of care, remove inconsistency and duplication for both staff and service users and a need to create a financially sustainable health and care system created a catalyst for significant and unprecedented change. Through Care Together, we want to create a system that works with the community to enable the population to avoid needing health care where possible, but is able to deliver care when it is needed, in a way that is seamless and delivered in the most appropriate place.
