Tender description
The service will be a support service for customers who are newly diagnosed with visual impairment/dual sensory loss, those who have lived their life with reduced sight loss and those who are deaf blind. The service will also meet the needs of those already in receipt of a Certificate of Visual Impairment who experience a change in their vision. The aim of the service will be to prevent customers from experiencing crisis that would require on going social care services the provider will promote the benefits of registering their sight loss to potential customers and the Rehabilitation Services available to them. This will require the Provider to work in collaboration with local hospital services and Eye Clinics to raise awareness of the support available across the County.
