Community Based Care Navigation Service - Prior Information Notice

Prior Information Notice: Ealing Clinical Commissioning Group wish to give prior notification of their intention to commission Community Based Care Navigation Service for patients registered with GPs in the borough. Care Navigation refers to assistance offered to patients and carers in ‘navigating’ through the complex health and social care systems to overcome barriers in accessing quality care and treatment and in particular accessing non-statutory services, working closely with the wider GP practice team and care coordinators. Invitation to Prior Information Notice event: Thursday 7th July 2016 at 11:00 in London. The Commissioner would like to invite interested potential providers to attend a Prior Information Event on Thursday 7th July at NHS Ealing Clinical Commissioning Group, 3rd Floor, Perceval House, 14/16 Uxbridge Road, Ealing, W5 2HL. Interested parties are requested to register for this event by 12:00 on Monday 4th July 2016 by email to: ealccg.cnproject@nhs.net The Event will provide opportunities to: — Understand the intention of the service; — Understand Commissioner's proposed future model of care; — Understand the forthcoming procurement process; — Meet and network with peers and/or potential partners. To ensure that the future Care Navigation Service provides the best all round outcomes for service users, the CCG is interested to consider the views and advise of those with experience in the delivery of Care Navigation Services. In all cases, the CCG alone will retain responsibility for deciding on the contents of all procurement documents. If the CCG decides to proceed with procurement, this is likely to begin in the middle of July 2016
