Tender description

The Integrated Public Health Nursing Service will provide leadership, co-ordination and delivery of the 0-19Healthy Child Programme (0-19 HCP) in Sefton. An essential part of the children’s services ‘system’, it will makea major contribution to our Children and Young People’s Plan ambitions by • Improving health and wellbeing outcomes for children and young people • Identifying additional needs early • Building resilience and • Reducing health inequalities by providing effective universal and targeted interventions for children and theirparents. The 0 - 19 HCP will be delivered to residents living in Sefton. All families with a child aged 0 - 5 years and allpregnant women currently resident in the local authority area must be proactively offered the HCP.
