Tender description

Care Navigation refers to assistance offered to patients and carers in ‘navigating’ through the complex health and social care systems to overcome barriers in accessing quality care and treatment and in particular accessing non-statutory services, working closely with the wider GP practice team and care coordinators. There are over 417 420 people registered with 79 GP Practices across the borough of Ealing; however the new service focus upon working with patients with long-term conditions to improve their quality of life and health outcomes.
