Tender description

This is the re-commissioning of a county-wide Healthy Child Service for 0-5 year olds. The key universal assessments offered by the Service include: Antenatal health visits; new baby review; 6-8 week assessments; 1 year assessment; and 2 to 2,5 year review. The aim of the Service is to promote health and development across the 6 high impact areas for Early Year's — Transition to parenthood and the early weeks — Maternal mental health (perinatal depression) — Breastfeeding (initiation and duration) — Healthy weight, healthy nutrition and physical activity —Managing minor illness and reducing hospital attendance and admission — Health, well-being and development of the child age 2-2,5 year old review (integrated review) and support to be ‘ready for school’. The Service requires flexibility to ensure that early intervention, community development and complex care packages are supported within the context of local needs.
