Tender description

Planning and Brokerage of assessed eligible social care needs into a Care and Support Plan. Subsequent and ongoing needs based support to people who take their authorised Personal Budget as a Direct Payment. A Support Planning and Brokerage Service — to help people define how their assessed eligible needs will be met, within their Indicative Budget. Subsequent and ongoing needs based support to people who take their authorised Personal Budget as a Direct Payment in the form of: ‘— A Direct Payment Set-up; — A Direct Payment Ongoing Support Service; — Employment Information and Advice Services; — An Employment Support Service (including a Payroll function); — For people who go on to employ a Personal Assistant; — A Support with Direct Payment Paperwork Service; — A Holding Account Service; — Whereby the Provider would make all payments to Personal Assistants, agencies and other providers and complete all the necessary paperwork to administer the Direct Payment on someone's behalf.’
