Tender description

This is an Award Notice. Southend-on-Sea Borough Council is inviting expressions of interest from a single (or single consortium) Provider for our Healthy Lifestyle Hub. Southend-on-Sea Borough Council are implementing a comprehensive programme of tailored lifestyle interventions and services for people living in Southend aged 16 years or over. The purpose of the Programme is to improve the health and well-being of the resident population in Southend and it will be delivered by a number of providers across a range of service areas, including: — Smoking Cessation and Tobacco Harm Reduction; — Weight management; — Physical Activity Interventions; — Health trainers (including workplace interventions); — Alcohol recovery; — Diabetes prevention; — Falls prevention / Postural stability; — Social prescribing / Mental health support / Emotional Health and Well-being; — Generic self-management; — Other programmes as required/commissioned by the Authority. The Provider will be responsible for the delivery of the ‘Gateway’ into the Programme, Health Trainer Service and for providing associated services as described in the tender document. The Provider will deliver an integrated single point of access to health improvement services, including a holistic assessment of an individuals need and signposting or referral into appropriate pathways before following them up to support maintenance of behaviour change. Delivery of the health trainer service will further support improvements to client's health and well-being with a specific focus on those at risk of health inequalities.
