Tender description

The PCT is looking for a modernised model of service which focuses on the provision of primary medical services to a targeted population of Care Home (Elderly) residents of Blackpool via a multi-disciplinary workforce. Blackpool has a high elderly population 14.2% aged 65-74, 10% aged 75-84 and 3.4% aged 85+ compared nationally with 8.3%, 5.7% and 1.9% respectively, and Blackpool also has a large number of Elderly Care Homes, (Private and Statutory). Residents of Care Homes present a number of additional problems; Increased workload for General Practitioners, Increased prescribing, Increased hospitalisation rates, Increased hospital bed usage, (sometimes inappropriate), Increased falls resulting in fractures, in particular neck of femurs and Increased mortality.
