Tender description

Lincolnshire County Council is undertaking pre-procurement market engagement in relation to the provision of a county-wide Well-being Service across Lincolnshire. This service shall form part of a wider Well-being Network; focusing on prevention and early intervention, and a reduction of throughput into health and social care services. The assessment will be done by the providers trained assessors who are skilled and trained to identify needs, establish how to meet those needs and either carry out the tasks themselves or organise others within the Well-being Service to do so. The generic support element is time limited to a maximum average of but not limited to 6 weeks support. It is a support service which will deliver what the individual requires to assist them to maintain their independence and potentially reducing overall need. Telecare will include initial installation and installation of additional items as required, programming, storage and stock control of equipment. The installation element will also include explanation and training for users/informal carers of the service so they feel confident in the use of the equipment provided through the service. This will also cover expert guidance on what additional equipment may assist or support an individual with certain issues. Where needed, battery changes and any changes required to ensure the system works effectively will form part of this element. Systems will be monitored 24 hours a day, 7 days a week (including bank holidays). The transport element on the Well-being Support Service is broken down into 2 sections namely Hospital Avoidance and Home-safe Transport.
