Enhanced GMS for Nursing Home Residents - Ealing PCT

NHS Ealing in conjunction with Ealing CCG are commissioning an “Enhanced Nursing Home Support Service”, this service will register nursing home clients and provide them with both core General Medical Services (GMS) as well as Enhanced General Medical Services. The enhanced service will offer a multi-disciplinary approach to pro-active case management, pharmaceutical care and urgent care “in-hours”. In Ealing there are 22 Nursing Homes with a capacity of around 1,147 beds (snap shot July 2012) which provide care for people with a range of physical, mental or emotional needs. In July 2009, NHS Ealing conducted an analysis of Ealing’s nursing homes which identified significant variation across nursing homes in the type and level of medical input received from General Practitioners and other healthcare professionals. The analysis also identified significant acute hospital related activity from nursing homes for conditions which could have been prevented, if good case management, appropriate nursing skills, medicines management and adequate policies, systems and protocols were in place. NHS Ealing recognises that the level of healthcare need among nursing home residents is complex and therefore calls for a level of care that is beyond essential general medical services. It is important that all Nursing Home Residents have equitable access to a quality and responsive local service. There are 2 distinct geographical clusters of Nursing Homes in Ealing, loosely correlating to the Southall, North of Ealing and Central Ealing & Acton.
