Please note: Although this project is under the heading ITT, it is to be used for distribution and submission of the Provider Accreditation documentation for the Glaucoma Referral Refinement Any Qualified Provider (AQP) procurement. Potential Providers are advised that this AQP scheme is being run on a local basis only. Providers who qualify under this scheme will not be accepted onto any nationally accredited Provider List. This phase of the AQP process is to enable potential Providers to access all relevant documentation and to facilitate submission of their completed Provider Accreditation Template. This opportunity is to determine a local list of Qualified Providers who can deliver the Service described below. Community based referral refinement of patients identified in a primary care setting, as being at potential risk of Glaucoma, (COAG (Chronic Open Angle Glaucoma or OHT (Ocular Hyper Tension)). The service listed will be located within the North East Essex PCT boundaries. Next steps for Potential Providers: 1. Download all attachments 2. Read the documents to understand the Commissioner requirements. 3. If you wish to become accredited as a Qualified Provider, complete the Provider Accreditation Template (PAT). 4. Follow the instructions in the Evaluation and Process Guidance Document to ensure you complete your response within the timescale. 5. If the Commissioner's requirements are unclear, use the Clarification Question Form to ask for further information. 6. Submit the PAT with all other supporting documentation using the 'Create Response' function within Bravo.
