Tender description

Supported Housing is focussed in the support that enables individuals to maintain their own tenancy, promote independent living and move on: usually people who live in supported housing have their support needs met by ‘housing related support’. Supported Living is more likely to be longer term help and support to enable a person to live as independently as possible. The Framework will be split into following lots as follows: Lot 1: Homelessness services; Lot 2: Multiple needs; Lot 3: Young people at risk; Lot 4: Learning disabilities; Lot 4a: Learning disabilities complex needs; Lot 5: Mental health; Lot 5a: Mental health complex needs; Lot 6: Physical and sensory needs. The framework will run from 10.6.2015 for an initial 3 (three) years with an the sole discretion of the Council, to extend for a further 1 year, subject to satisfactory contract performance and the Council having the need for a provider framework and the funding available for the framework. These support services are for homeless households, families and individuals aged 16 years old or older who are experiencing homelessness or who are at risk of becoming homeless and are in priority need that fall within the definition of the Housing Act 1996 (part 7) where they also meet the requirements of the Bradford Eligibility Criteria for Housing Related Support. The service(s) will operate short term support which promotes early move on and therefore a greater throughput into independent sustainable living. The provision in this Lot will include: — Accommodation based provision with 24hr support on-site; — Resettlement floating support provision; — Out-of-hours homeless advice; — Street outreach. The aim of the service(s) to be tendered through this lot will be to provide accommodation based and floating support to clients in need of housing support who have other multiple needs in addition to being homeless or at risk of homelessness. In particular client groups that have co-existing or additional needs (for example, people with mental health and substance misuse problems, personality disorders, challenging behaviour, physical health problems, offending behaviour). Generally people with 3 or more conditions will fall into this cohort, however, it is also recognised that a person may have such significant support needs in relation to any 1 of the above that they could be considered as falling into the multiple needs category, the housing assessment team will make these individual assessments. Supported living services and supported accommodation services can be delivered through accommodation based services, or through floating support services. Accommodation based services are linked to a property or scheme, such as self-contained accommodation, and floating support is provided in a persons own home. If the individual moves house, the floating support can move with them. The services included in this tender all provide Housing Related Support only, and are typically short-term therefore the general route is for people to move-on to independence. The provision in this Lot may include: — Accommodation provision with 24hr on-site support; — Foyer provision with sleep in cover provided; — Move on accommodation with day time support on site and on-call support; — Floating support to provide resettlement support and support to prevent tenancy breakdown. The Council and health services who have assessed and approved access to services under this lot will have identified an individual who has a recognised learning disability and is in need of support services, this could range from floating support service into a persons home, to providing 24 hour support, 24/7 in shared accommodation. It is the Council's intention, wherever possible, to ensure the support provider is not also the accommodation provider/landlord. Therefore, this Lot will primarily be used to procure the support people require. The supply of housing and the associated housing management functions will be provided by Housing Providers. This lot is to recognise needs of people with learning disabilities and complex needs, including those on the autistic spectrum, and those with complex presentations. This will further include people with complex learning disabilities in addition to other prevailing conditions which mean independent living is achieved with support. The Council wish to have a range of support services and in certain circumstances may require the support provider to also provide accommodation; these services range from individual properties with individual tenancies with support to keyring core and cluster models of support. Innovative solutions will be welcomed in terms of service design. The Council needs to ensure it has a variety of quality services available to meet the needs of this client group. People with severe and lasting mental health problems with additional complexities, including having a forensic history and those who challenge traditional services, meeting the prevention agenda in terms of hospitalization and re-admissions, support will range from: — Low level floating support and outreach; — To 24/7 support services. The council wishes to develop its variety of provision especially in the area of complex need, for significant mental health issues including autism where behaviours challenge standard support services. In the UK the notion of ‘challenging behaviour’ is frequently adopted as a means of describing people whose behaviour presents a significant challenge to services, whatever the presumed cause of the issue. The Council needs to ensure it has a variety of quality services available to meet the needs of this client group. People with severe and lasting mental health problems with additional complexities, including having a forensic history and those who challenge traditional services, meeting the prevention agenda in terms of hospitalization and re-admissions, support will range from: — Low level floating support and outreach; — To 24/7 support services. The council wishes to develop its variety of provision especially in the area of complex need, for significant mental health issues including autism where behaviours challenge standard support services. In the UK the notion of ‘challenging behaviour’ is frequently adopted as a means of describing people whose behaviour presents a significant challenge to services, whatever the presumed cause of the issue. Supported living will also include a lot specifically for physically disabled and sensory disabled citizens, the Framework Agreement looks to engage with experienced providers in this area, with trained staff familiar with working with and supporting citizens in this area of need. This will include those having physical disabilities which severely affect the ability to be independent, having sensory disabilities which severely affect the ability to be independent or having a combination of physical and sensory disabilities. In particular, the Council wishes to enhance its provider market by having more providers specialising in this area of support.
