Support to live at Home contract and Extra Care Housing contract Round 3

Tender description

The work we are tendering for will be: twenty four hour home care provision for older and disabled people (including those funded by Health for ongoing long term health conditions); end of life care at home; dementia care; reablement; and provision of home care at one extra care housing scheme. The work does not include: provision for people with a learning disability; provision for people with poor mental health who are of working age; or provision for people funded by health and social care due to complex needs. The Support to Live at Home contract will be a spot purchase contract. The geographical area in question for the delivery of home care comprises: • Kingston Seymour • Clevedon • Portishead • Pill • Abbotts Leigh • Leigh Woods There are approximately 2500 home care hours delivered in this area currently per week. The Extra care scheme included in this tender is: Waverley Court Extra Care Housing, 68 Fourth Avenue, Portishead, BS20 7NY - 60 one and two bedroom apartments, 45 of which rented via North Somerset Council, approximately 380 hours of home care per week anticipated and 1 waking night, 7 nights per week for a client with learning disabilities in addition to the included waking night cover.
