Tender description

Contract for the provision of a NHS Health Checks Service. The contract will commence on 1.1.2017 and will be for a 3 year period, making the initial expiry date 31.12.2020. There is an option to extend the contract by up to a further 2 years making the final possible expiry date 31.12.2022. Any extensions will be offered at the sole discretion of Wakefield Council, subject to funding, performance and local need. Contract for the provision of a NHS Health Checks Service. 1 Service provider will be appointed to deliver the Service through a single contract and must demonstrate within their tender return how they will structure the Service to meet the outcomes and objectives outlined in the specification.
