Contract for a maximum period of 3 years (2 years + optional 1 year extension) for the provision of domiciliary care and support services to individuals living in supported accommodation at the following: — 123 Segensworth Road, Titchfield, Fareham, PO15 5EG. — Moss Cottage, 7 Western Road, East Liss, Liss, GU33 7AG. — 12 Dormy Way, Gosport, PO13 9RF. following de-registration of these properties as registered care homes and the grant of tenancies to the individuals residing within the properties. The contract was awarded without prior publication of a contract notice due to the contracting authority wishing to facilitate the de-registration of residential care homes and provide its service users with increased independence and security in their existing accommodation where domiciliary care and support services will be provided to them under the contracting authority's contract.
