Tender description
This contract has 2 lots. Lot 1 is a hospital discharge and reablement service This service will take all hospital discharge referrals and assess whether they require reablement. If eligible a reablement package will be provided for up to 6 weeks. The service will also provide a welcome home service for vulnerable individuals who do not require reablement. Lot 2 is a community reablement service This service will provide up to six weeks reablement for those who are living in the community. Keywords: reablement
